Launch of the ‘Guide to Accessing Services’
Health, local government and community service providers collaborating and working in partnership to improve health outcomes and provide a more efficient health service system for the community in Melbourne, Moreland, Moonee Valley and Yarra.
Primary Care Partnerships (PCPs) began in 2000 as an initiative of the Victorian Government. PCP membership typically includes acute and community health, aged care assessment services, aboriginal health, ethno- specific services, local government, mental health, divisions of general practice, women’s health, nursing services, homelessness services, alcohol and other drug services, disability services, palliative care, sexual assault services, dental health and many other services.
Following the Minister for Health’s decision in late 2009 to realign PCP boundaries in the North West Metropolitan Health Region, the new Inner North West Primary Care Partnership was established on 1 July 2010. One of the first tasks for the members of the Inner North West PCP is to develop a strategic plan that will focus on local well being priorities including one or more chronic health conditions.
In addition, this strategic plan aims to strengthen the catchment planning response to health inequalities and social determinants of health while addressing the barriers to health service participation and improving access to health services for the vulnerable and hard to reach groups e.g. socio economic disadvantaged groups, indigenous groups and those experiencing complex health issues and or chronic disease. Importantly, the Primary Care Partnership’s collaborative approach to health promotion, service coordination and integrated chronic disease management will serve to keep local communities in Melbourne, Moreland, Moonee Valley and Yarra healthier.
