Support and Choice Program
Support and Choice is a Department of Human Services initiative which uses the principles of individualised planning and support. The program involves people with a disability and their families developing a tailored plan to meet the individual’s goals. The Support and Choice Program aims to improve the quality of life of those who are chosen to participate.
A Support and Choice plan will cover all areas of your life including community access, your friendships and relationships and will assist you to access formal support if required.
Support and Choice Facilitator
A Support and Choice Facilitator helps people to develop an individualised plan. If you have been selected by Department of Human Services to participate in the Support and Choice program you will be asked to select an agency to help facilitate the development of your plan.
A CarerLinks North Support and Choice Facilitator will work together with you and your family and friends (if appropriate) to;
Eligibility
If you would like more information about the program please call your local Department of Human Services Intake and Response service on 1800 783 783. They will assess your eligibility and take a referral. If you are selected to participate in the program you can then choose MCHS – CarerLinks North to assist you to develop your plan, if you wish.
