Planned Activity Groups

Engaging and fun group activities to help older people and people with disabilities stay active.

Planned Activity Groups (also known as adult day activities centres) are designed to help older people and people with disabilities stay healthy and active by engaging in interesting and fun group activities.

Activities include physical exercise (such as tai chi and walking), arts and crafts sessions, discussion groups and talks about ways to improve health and wellbeing.

Planned Activity Groups also function as respite, allowing carers time out to catch up on other tasks. For more information on services for carers, see CarerLinks North.

 

Planned Activity Groups provide an opportunity to:

  • Continue to enjoy old hobbies and pastimes
  • Make new friends and develop new interests
  • Keep your body and mind active
  • Work on skills needed for living independently in your own home

 

Planned Activity Groups are also a great way to stay informed about health and community services that may be of benefit to you from time to time.

 

Planned Activity Groups at MCHS

MCHS operates over 24 different Planned Activity Groups per week. Some groups focus on getting out and about while other groups meet at one of our 4 local venues to participate in exercise classes, art and craft activities and discussion groups. We also run groups for residents of Supported Residential Services (SRS). With such a range of groups on offer, there is bound to be one which meets your needs and interests. Groups run between the hours of 10am and 3pm Mondays through to Fridays.

For most groups, our purposely modified bus can transport you to and from the group and our friendly cook ensures all group members enjoy a warm and tasty meal.

The majority of our groups are funded through the Home and Community Care Program (HACC), while two are funded through the National Respite for Carers Program (NRCP)

 

Choosing a Group that’s right for you 

For more information or to join one of our groups please contact the Assessment Coordinator on (03) 9384 2244. The Assessment Coordinator will explore with you which group best meets your needs and interests.

 

Joining a Group

Once we have agreed on a suitable group, the Team Leader will contact you to arrange your first visit. Sometimes attending a new group can be daunting. Our Team Leaders are here to answer any questions and ensure you feel welcomed and supported on your first few visits. If you are feeling a bit uncomfortable about your attending, the Team Leader can arrange for you to come in for shorter visits until you feel confident to stay for the whole day.

 

Referrals

Referrals to Planned Activity Groups can be made by:

  • Individuals
  • Family members and carers
  • Doctors or Specialists
  • Other health professionals
  • Other service providers

 

Referrals can be made by:

  • Calling the Assessment Coordinator on 9384 2244
  • Faxing the Service Coordination Tool Templates (SCTT) to the Glenroy office on 9300 3283

Eligibility

You may be eligible to join one of our groups if you are frail, have a disability or find it difficult to attend groups and clubs independently.
