Occupational Therapy services provide assessment and advice to help people achieve maximum mobility and comfort at home.

Occupational Therapy (OT) services assist frail aged and disabled people to stay in their homes safely. Our Occupational Therapists provide assessments in the home and arrange for services and equipment to promote independence, and enable the client (and their carer) to improve their ability to manage daily activities. The focus is on safety and enhancing independence.

The Occupational Therapists can arrange for clients to obtain a wide range of equipment such as grab rails, bath/shower seats, chair raisers, hospital style beds and commodes. Major or minor home modification plans are developed in conjunction with an architect and can include bathroom conversions and ramp access for clients requiring wheelchair access. The Occupational Therapist will also advise on funding subsidies for the purchase of equipment/modifications and can assist in the application process. A small store of equipment is available for loan. 

 

Wheelchair/Pressure Care

A specialist service exists for wheelchair, pressure care and seating prescription for clients with complex care needs.

 

The Falls Prevention Service

This service aims to provide screening, information / education, advice and interventions that target clients at risk of falls. This includes referral to groups and other falls prevention services, provision of equipment and assisting clients to identify their risk factors and formulate self management plans.

Referrals are accepted from any source.

 

Eligibility

Residents of the City of Moreland who are:

  • Frail aged
  • Have a disability
  • Have a long term illness and complex care needs
  • Palliative Care clients
  • Clients covered by TAC, Work Cover (i.e. compensable clients) or clients resident in nursing homes are not eligible.
