Community Health Nursing Information Sheet

 

A range of nursing services to encourage good health.
A number of different services and programs are offered by our nurses. These include:

  • Women’s health – individual assessment or education and training
  • Men’s health – individual assessment or education and training
  • Drug and alcohol – personal assessment and service co-ordination
  • Health assessments
  • Aged care support
  • Health promotion and prevention initiatives through education and screening sessions
  • General health support, counselling and advocacy
  • Referral to other programs, services or health professionals as appropriate.

 

Eligibility

  • Referral for community health nurses programs and services can be made by individuals (selfreferral), carers, health professionals including doctors and other service providers
  • For information on the specific criteria of individual programs call the Service Access team.

 

Fees

Session fees are $12.50 per session, $8 per session
for Health Care Card and Pension Card holders.
