Commonwealth Carelink Centres aim to help you live independently in your own home. Anyone who needs information about community aged care, disability and other support services can call us on 1800 052 222. We can provide advice on local services that may be able to assist you.

Information is available on;

  • the range of services available and how to contact them
  • who is eligible to receive the services
  • whether there are any costs associated with receiving the services, and
  • aged care assessment services for access to Community Aged Care Packages or entry into aged care homes

The types of services that we can provide you with information on include;

  • household help, home modification and maintenance
  • personal, nursing and respite care
  • transport and meal services
  • disability services
  • day care and therapy centres
  • assessment, including Aged Care Assessment Teams
  • special services for dementia
  • continence assistance
  • support for carers
  • Community Aged Care Packages
  • aged care homes
  • range of allied health care (eg. podiatry, physiotherapy, etc...)
  • support groups

 

The Commonwealth Carelink Centre information service can be used by anyone, including older Australians, people with disabilities, their families and carers, general practitioners and other service providers.
