Support to keep frail aged persons and people with disabilities living with complex and multiple support needs to remain in their own homes and local community.
Case Management Services operates across the municipalities of Moreland and Hume and provides support to frail older people, people with dementia, younger people with disabilities who have complex and/or changing care needs and their carers. At all times the program works to enhance the dignity, independence and autonomy of clients.
The programs are based on a case management model and are funded through the Home & Community Care (HACC) initiative and the Commonwealth government under the Community Aged Care Program (CACP). Both services are designed to enhance, or provide a top up to the service already provided by other agencies.
The Linkages Program is available to 129 frail aged people and younger disabled people who reside in the Moreland and Hume local government area and are at high risk of being admitted to residential care.

When a program place becomes available, the ultimate decision regarding acceptance remains with the program. An appeal mechanism is available upon request.
