Clients are expected to contribute towards the cost of their care using the Community Packaged Care guidelines or HACC Fees Policy Guidelines.

Support to keep frail aged persons and people with disabilities living with complex and multiple support needs to remain in their own homes and local community.

Case Management Services operates across the municipalities of Moreland and Hume and provides support to frail older people, people with dementia, younger people with disabilities who have complex and/or changing care needs and their carers. At all times the program works to enhance the dignity, independence and autonomy of clients.

The programs are based on a case management model and are funded through the Home & Community Care (HACC) initiative and the Commonwealth government under the Community Aged Care Program (CACP). Both services are designed to enhance, or provide a top up to the service already provided by other agencies.

The Linkages Program is available to 129 frail aged people and younger disabled people who reside in the Moreland and Hume local government area and are at high risk of being admitted to residential care.

Of the 129 places:

  • approximately two thirds are for residents of Moreland and one third for Hume residents
  • approximately two thirds of places are designated for frail aged people and the other third for younger people with disabilities and complex care needs
  • a percentage of places have a brokerage (purchase of service) component, whilst others provide Case Management/Care Coordination only.
  • The Community Aged Care Program is available to 15 frail aged people who reside in the Moreland and Hume local government areas and are at high risk of being admitted to residential care.
  • Of the 15 places:
  • approximately 1 third  are for people from a CALD background
  • approximately 1 third of places are designated for people from socially and financially disadvantaged backgrounds

 

Eligibility

The assessment and case planning process will determine an individual's eligibility for assistance by this service.

 

For Linkages

Individuals will be eligible for inclusion into the service if they:

  • reside in Moreland or Hume
  • wish to remain in their home
  • require ongoing case management to plan, coordinate, monitor and match services to care needs
  • live alone, or have a carer who needs support to maintain their caring role
  • have complex needs and need more flexible and/or extensive services than are currently available
  • need services/resources currently not available through other programs
  • require support/advocacy/monitoring to remain at home
  • client suitability for the program
  • For Community Aged Care Program

 

Individuals will be eligible for inclusion into the program if they:

  • Have been approved for the level of care by an Aged Care Assessment Service
  • reside in Moreland or Hume
  • wish to remain in their home
  • require ongoing case management to plan, coordinate, monitor and match services to care needs
  • live alone, or have a carer who needs support to maintain their caring role
  • have complex needs and need more flexible and/or extensive services than are currently available
  • need services/resources currently not available through other programs
  • require support/advocacy/monitoring to remain at home
  • client suitability for the program

When a program place becomes available, the ultimate decision regarding acceptance remains with the program. An appeal mechanism is available upon request.

 

In assessing priorities and allocating places in the service, consideration will be given to the following client risk factors/situations:

Clients who;

  • are living alone, or living with a frail or disabled carer
  • are confused
  • are at risk due to stress or incapacity of carer
  • have no family/carer support
  • have high level of dependency/disability with complex and / or changing care needs
  • have special needs due to cultural differences

Acceptance in to the service is dependent on funds being available and eligibility criteria being met.
