Avalon Outreach Program

One to one psychosocial rehab recovery support for adults with a mental illness who live independently in the City of Moreland.

Avalon is a support program that provides a high quality outreach service to maximise choice, consumer rights and involvement. Participants are encouraged to live as independently as possible within the community.

An individual program is completed with participants to assist them to set individual goals and take agreed action to achieve them.

The Avalon Outreach Program is part of the non-clinical specialist mental health services funding by the Department of Human Services.

Referrals
Participants can get involved by contacting the Intake Worker for either program. We accept referrals from health professionals or self-referrals.

 

Eligibility

The Avalon Outreach Programs provide services for people who:

  • have a diagnosed mental illness,
  • live in the City of Moreland are aged between 18 and 65,
  • have the ongoing support of a clinical health professional,
  • are willing to participate in the programs.
