Service Access

The Merri Community Health Services, Primary Health Care Program is grouped into three teams; the Independent Living Team, the Wellness and Preventative Care Team and the Chronic and Coordinated Care Team. The program offers a range of health practitioner services such as:

  • Community Nursing – a range of programs and support delivered by our nursing staff
  • Diabetes Education – advice and education for those with diabetes
  • Dietetics  – healthy eating advice and nutritional assessment for people with a diet related health issue
  • Occupational Therapy – advice and assistance to make it easier for people to perform their daily physical activities 
  • Physiotherapy – assistance with movement and mobility difficulties, injuries and/or pain conditions
  • Podiatry – prevention, diagnosis and treatment of foot and lower limb conditions
  • Speech Pathology – assessment and management of communication and swallowing problems
  • Referrals to most of these services can be made by individuals (self-referral), carers, health professionals including doctors and other service providers

Fees

  • Session fees for many of our programs are $12.50 per session, $8 per session for Health Care Card and Pension Card holders. But see specific program pages for further details.

Independent Living is funded under the Department of Human Services' Home and Community Care Program.
